Please use the new version of the Signature Authorization form (GAO-3 rev 04/2008) that is on the General Accounting Office (GAO) Web site. The form is available on the Online Forms page (http://www.gao.az.gov/onlineforms/) or click here.
Click the “Open Instructions” box in the upper left corner of the Signature Authorization form to view (or print) instructions for filling out the form.
The agency head uses the Signature Authorization form to delegate signature authority for various types of documents to responsible individual(s) within the agency. The form is required to verify signatures on documents sent to the General Accounting Office (GAO) for approval and/or processing.
With the new GAO-3, the agency head will delegate signature authority to one employee per form.
Please submit a new Signature Authorization form to GAO Security by September 30, 2008, for each currently authorized employee who was granted signature authority with a prior version of the form. The new form requires additional information.
GAO Security will stop accepting prior versions of the Signature Authorization form after July 11, 2008. Please delete old electronic templates saved on your computer and recycle paper stock of older versions of the form (10/2003, 06/1997, and 10/1988).
The form has been revised to make its proper completion easier, to facilitate review and maintenance of agency signature authority, and to provide stronger controls on transactions that are approved and/or processed at the GAO.
The new form captures the Arizona Department of Administration (ADOA) Human Resource Information Solution (HRIS) Employee Identification Number (EIN) and other information to help the agency and GAO staff to identify the employee. This will help the agency security administrators and GAO staff to ensure that the agency’s signature authority is appropriate, and to identify when an employee changes position or leaves the agency. It will also help GAO staff to contact the employee, security administrator(s), or agency head, if necessary, to ensure the timely processing of documents submitted to the GAO.
The new Signature Authorization form requires special software, Adobe Reader 8 or higher, that allows you to save a PDF file with the data you entered on the form. A link for a free download of Adobe Reader 8 or higher is available on the GAO Web site Online Forms page or click here.
