Four more forms—the GAO-3, GAO-508, GAO-516 and GAO-615—suitable for download and reproduction by agencies, have been added to the GAO Website.

In its continuing efforts to deliver more products and services to State Agencies electronically, the GAO has expanded the list of forms available on-line. The newly added forms, which can be downloaded and reproduced by Agencies as needed, are:

GAO-516-Payroll Transaction
GAO-615-Payroll Stuffer Request
Making forms available electronically saves the State money and allows for more frequent modifications of forms as circumstances require; it also means that State agencies have convenient and ready access to the forms they need to conduct their business.
