The new form:
Clarifies that the form is not optional, but required by State policy.
Utilizes condensed language regarding attaching bank statements showing account/routing numbers and specifically allows statement photocopies.
Includes the added language “I certify I am an authorized signer for any account(s) I am adding or changing”
Indicates that the Payroll Card will be the default payment method if no direct deposit for an established bank account is designated.
Clarifies that country of residency and citizenship are required for a Payroll Card account to be established.
Includes an option box and agency signature line for agency payroll to use if the employee is being involuntarily signed up for a Payroll Card.
Has removed the Central Payroll fax number and email since employees were sending to Central Payroll instead of their agency payroll for processing first.
