Form GAO 73 (Authorization to Start, Change, or Stop a Voluntary Deduction) updated.

The following deductions codes will no longer be active or available after the July 20, 2012 pay period end date:
          o 3127 DUES - AFSCME
          o 3129 INS – AFSCME
          o 3137 DUES – AZCOPS
          o 3143 DUES – SEIU

Additionally, any existing active deductions in the system for current employees will be end-dated after the July 20, 2012 pay period end date.

Please use the new current form GAO 73 available on the GAO website at: http://www.gao.az.gov/onlineforms/default.asp and discard prior versions.
